Why Measurement Quality Matters

Evaluating the tools we use to measure outcomes is just as important as choosing the outcomes themselves.

In mental health research and care, especially for children and youth, we rely on outcome measurement instruments (OMIs) including patient-reported outcome measures (PROMs) to understand what’s working and what’s not. But not all tools are created equal. Some may be too long, too vague, or not meaningful to the people they’re meant to help.

That’s where the COSMIN framework comes in.

What is COSMIN?

COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) is an internationally recognized framework developed by experts in psychometrics, epidemiology, and clinical care. It provides a rigorous, transparent process for evaluating whether a measurement tool is:

Reliable – Does it produce consistent results?

Valid – Does it measure what it’s supposed to measure?

Feasible – Can it be used easily in real-world settings?

Why Measurement Matters for Youth Mental Health:

Without strong measurement tools, we risk drawing the wrong conclusions from clinical trials or care programs. This can lead to:

• Ineffective or harmful treatments

• Missed opportunities for improvement

• Wasted resources and research fatigue

By applying the COSMIN Framework, we ensure that the measurement tools we use are not only scientifically sound, but also meaningful and practical for youth, families, and clinicians.
